New Patients
If your pet will be coming for his or her first visit, please download and bring the completed new patient registration form to your appointment and/ or fill out the online paperwork. If your pet has a previous veterinarian, please have his or her records emailed to us at info@lahny.com prior to your appointment. We are required under NY State law to vaccinate for rabies. If you will be bringing your records with you, we kindly ask that you arrive 10 minutes prior to your appointment time to allow us time to copy and review your pet’s medical history.
